Our preference is to serve our clients with the best possible care. As we learn about your story, we require certain pieces of information to guide us along the way. Our History Form provides key indicators of family and incidental factors crucial to understanding the client as a whole individual. As a Counseling Center, we are also required to obtain information for insurance purposes including the Informed Consent Form and for those who require special arrangements for children, the Custody Addendum Form.
Our staff treat all clients equally regardless of type of insurance, race, ethnicity, gender, religious preferences, or sexual orientation. In best practice, we also offer a Coordination of Treatment opportunity - requiring additional information from our clients and is necessary for compliance for all Medicaid clients.
The Wabash Friends Counseling Center offers a Sliding Scale payment options for services not covered by insurance with the exception of Drug & Alcohol Assessments, counselor requested court appearances, and report writing. To qualify for the Sliding Scale payment option, please submit your three most recent paycheck stubs or advises to the Wabash Main Office.
